Nursing Services

Nurses strive to support individuals by preventing illness, and promoting health, which is a state of physical, mental and social wellbeing.  health is not just the absence of disease or infirmity.

Nurses can provide individuals with the knowledge and skills to understand, evaluate and apply relevant information to make effective decisions about health matters and then take appropriate action.  We know that when nurses are involved in supporting the functional independence and quality of life of people within our community, we can help people to remain in their homes longer.

Medication Management

We can assist with any medication needs you may have, including aadministration or prompting.  A nurse can monitor the effectiveness of your medication and liaise with your medical team if required.

Dementia Care

The care provided by our nurses and support partners includes education and family support, validation and reminiscence therapy, behaviour management, assessments, referrals and therapeutic support

Wound Care

Nurses can assist with supporting your skin integrity and use evidence-based practices to assist with the management and treatment of wounds in the comfort of your home.

Chronic Conditions 

Our nurses care for people with diabetes, heart, lung, kidney and other concerns.  We can provide health monitoring in your home or remotely with education and oversight by a clinician who will be notified of any concerns.

Palliative Care

In partnership with others involved in your care, we will support you to ensure your comfort, any symptoms or concerns are well managed and your wishes are known and respected.

Continence & Catheter Care

Urinary incontinence affects men as well as women and is not a normal part of ageing and can be treated.  Our nurses can assist with any bowel or bladder management strategies, assessments, reviews, and catheter care.
